Health Choice – What Is It?

Medical insurance for children up to 19 years of age and pregnant woman of any age.

What Is Covered?

For pregnant women:

  • Pre-natal and post-partum doctor visits
  • Lab tests
  • Prescriptions
  • Doctor and hospital delivery fees
  • Home visits
  • Mental Health
  • Drug & Alcohol services
  • Routine vision and dental visits
  • Emergency services
  • Transportation
  • For infants and children up to age 19:
  • Sick and well visits to the doctor
  • Hospitalization
  • Immunizations
  • Prescriptions
  • Routine dental visits
  • Vision services
  • Mental Health
  • Drug & Alcohol services
  • Transportation

Who Is Eligible?

Must be a Maryland resident
Children (newborns up to age 19)
Pregnant women (any age)
Household gross income that meets or is less than certain qualifying levels.

Note: If you have health insurance or have recently dropped health insurance, you may not be eligible for this program.

Call for or pick up an application at the Carroll County Health Department at 410-876-4960 or 410-876-4916.

Those found eligible for this program will have to choose a Managed Care Organization (MCO) and will receive an enrollment packet in the mail.
