Behavioral Health: State Care Coordination

Prisoners preparing to be released from jailState Care Coordination offers support services for adults with a substance use disorder (SUD) who are transitioning into the recovery community from SUD treatment or the local detention center, or are experiencing homelessness.Care Coordinators assist patients by providing a connection to community- or faith-based services and other human services organizations. State Care Coordination services are designed to help individuals remain engaged in their recovery and to promote independence and self-sufficiency.

How to Obtain Services

Who can get services?

Adults 18 and older with a substance use disorder (SUD) or co-occurring mental health and substance use disorder who are transitioning into the recovery community from SUD treatment or the local detention center, or are experiencing homelessness. Must be a Maryland resident.
