Projects for Assistance in Transition from Homelessness (PATH) Program

PATH Program helps connect adults 18 and over who are experiencing homelessness, and who have been diagnosed with a serious and persistent mental health disorder, to link to benefits, entitlements, housing, long-term treatment and supportive services as needed.  Case management and peer support are offered through this program.

How to Obtain Services

Who can get services?

Homeless people with serious mental illness (substance use disorders can be co-occurring) who are not insured, on Medical Assistance (MA), or on Medicare.
